Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality nursing care to patients at the Equitas Health Medical Center. As a key member of the multidisciplinary team, the LPN will work collaboratively with healthcare professionals to deliver comprehensive medical care to patients living with HIV/AIDS.

Responsibilities
  • Provide direct nursing care to patients, including phlebotomy, immunizations, injections, and health maintenance screenings.
  • Follow approved nursing and clinic protocols and procedures to ensure the highest level of patient care.
  • Triaging patient calls and arranging sick visits as necessary.
  • Maintain a nursing message line and respond to messages in a timely manner.
  • Prepare exam rooms for patient visits and ensure infection control procedures are followed.
  • Document all care provided in the electronic medical chart.
  • Process patient referrals for specialty care and follow up as needed.
  • Communicate with internal and external case managers regarding patient care and referrals.
  • Provide education to patients on HIV disease, medical care issues, and adherence strategies.
  • Develop a strong collaborative relationship with the HIV medical community to enhance patient access to services.
Requirements
  • Active and unsanctioned license as a Licensed Practical Nurse.
  • Certification in CPR/BLS.
  • Three years or greater experience in a medical setting.
  • Thorough knowledge of nursing practice standards.
  • Ability to be flexible in work hours.
  • Exemplary customer service and written and verbal communication skills.
  • Knowledge of health and social service needs of persons with HIV/AIDS.
