We are seeking a compassionate and skilled RN Case Manager to join our Hospice team at Assured Home Health & Hospice.

About the Role

As a Hospice RN Case Manager, you will be responsible for delivering high-quality end-of-life care to patients and their families. You will work closely with the interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care and support.

Key Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify patients' physical, psychosocial, and environmental needs, and reassess as needed, no less than every 15 days.
  • Initiate and coordinate care plans, maintaining accurate and relevant clinical notes regarding patients' conditions.
  • Document problems, goals, interventions, and patient/family response to hospice care.
  • Collaborate with patients, families, attending physicians, and other members of the IDG to provide daily care and support.
Requirements
  • One year of clinical experience.
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
