We are seeking a compassionate and skilled Registered Nurse to join our team as an On-Call Registered Nurse. As a member of our hospice care team, you will provide high-quality patient care and support to individuals and families in their homes and communities.
Job Summary:The On-Call Registered Nurse is responsible for organizing and directing hospice care, with a focus on community health education and experience. This role requires a strong nursing background, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities:- Provide comprehensive assessments and evaluations of patients and families to determine hospice needs.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Respond to after-hours calls and make visits to patients with changing conditions, completing weekend admissions as needed.
- Initiate and revise care plans as necessary, utilizing health assessment data to inform nursing diagnoses and interventions.
- Administer medications and treatments as prescribed by the physician, and provide health education to patients and families.
- Supervise ancillary personnel and delegate responsibilities as required.
- Participate in quality assessment and performance improvement activities to ensure high-quality patient care.
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care.
- Current licensure in the state and CPR certification.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
