Job Summary

We are seeking a highly skilled and compassionate RN Case Manager to join our team in Saline County, Kansas. As a Field-Based RN Case Manager, you will play a critical role in assessing, planning, and implementing care strategies for Medicare and Medicaid members. Your expertise will be instrumental in ensuring that our members receive high-quality, patient-centered care.

Key Responsibilities
  • Assess and plan individualized care strategies for members with complex medical and behavioral needs.
  • Utilize strong communication and interpersonal skills to engage members in their care and promote self-management.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions.
  • Develop and implement care plans that address social determinants of health and promote health equity.
  • Provide education and support to members on disease management, medication adherence, and healthy lifestyle choices.
  • Identify and initiate referrals for community resources and social services to support member needs.
Requirements
  • Current, unrestricted RN license in the state of Kansas.
  • 2+ years of experience working in community-based settings, such as acute inpatient care, long-term care, home health, or hospice.
  • Intermediate level of proficiency in MS Word, Excel, and Microsoft Outlook.
  • Ability to travel up to 75% of the time with reliable transportation and a valid Kansas driver's license.
Preferred Qualifications
  • Bachelor's degree in nursing.
  • Experience working with Medicaid and Medicare populations with complex medical and behavioral needs.
  • Background in managing transitions of care and coordinating community resources.
