About the Role

We are seeking a compassionate and skilled Home Health Physical Therapist to join our team in Voorhees, New Jersey. As a per diem / prn Physical Therapist, you will provide high-quality patient care, promoting independence, comfort, and dignity for patients in their own homes.

Job Description

This is a flexible, weekend position that requires active listening skills, excellent assessment skills, and strong written and verbal communication. The ideal candidate will have a calm and reassuring presence, with the ability to deliver patient care as they would want those they love to be treated.

You will use your clinical skills to treat patients where they want to be the most – in the comfort of their own home. Your responsibilities will include:

  • Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
  • Developing and implementing appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate.
Requirements

To be successful in this role, you will need:

  • An active and current license to practice as a professional Physical Therapist (PT) in the state of NJ.
  • Previous home healthcare experience welcomed, but not required. We will train you.
  • The ability to travel to cases as assigned.
  • A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
