$85,000 - $115,000 per year

About the Job:

This role is responsible for planning and coordinating patient care services to meet the unique needs of each individual. You will work closely with patients, families, and interdisciplinary teams to develop and implement comprehensive care plans that promote optimal health and well-being.

Your primary focus will be on ensuring that patients receive high-quality, compassionate care that meets their physical, emotional, and social needs. You will also collaborate with other healthcare professionals to identify areas for improvement and develop strategies to enhance patient satisfaction and outcomes.

Main Responsibilities:
  1. Develop and implement patient-centered care plans that address the physical, emotional, and social needs of each individual.
  2. Collaborate with interdisciplinary teams to coordinate patient care services and ensure seamless transitions between settings.
  3. Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
  4. Participate in quality improvement initiatives to enhance patient satisfaction, safety, and outcomes.
What We're Looking For:
  • BSN degree or equivalent required.
  • Current RN licensure in good standing.
  • CPR/BCLS certification.
  • ACLS certification preferred.
  • Minimum 6 months of experience in a hospital or healthcare setting.
