Job Summary

We are seeking a skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare staff, you will be responsible for delivering exceptional patient care, collaborating with our medical team, and making a meaningful difference in the lives of our patients.

Responsibilities:
  • Deliver High-Quality Patient Care: Ensure that all activities performed align with our organization's vision, meeting the needs of patients and their families in their homes.
  • Collaborate with Medical Team: Work closely with our medical director, attending physician, and branch leadership to ensure seamless communication and effective patient care.
  • Prevent Unwanted Hospitalizations: Identify and address potential health issues early on to prevent hospitalizations and ensure patients receive the right care at the right time.
  • Maintain Accurate Records: Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
  • Comply with Regulations: Ensure compliance with applicable laws and regulations, including Collaborative Agreements.
Requirements:
  • Masters Degree in Nursing: Possess a Master's degree in nursing and certification through a national Nurse Practitioner Licensing board.
  • Clinical Experience: Have 3+ years of clinical nursing experience, with 1+ year post-graduate experience in hospice and palliative care.
  • Current Licensure: Maintain current licensure as a Nurse Practitioner in the state of practice.
  • Board Certification: Preferably hold Board Certification in Palliative Care.
  • Active DEA License: Possess an active DEA license.
  • Travel Requirements: Able and willing to travel up to 50% as needed to support business and patient needs.
