Springfield Medical Care Systems, Inc. is seeking a highly motivated Community Health Worker to join our team. This role will play a critical part in helping individuals access the care they need through screening, brief interventions, and connections to services.

Key Responsibilities:
  • Promote and achieve our mission to connect with and care for our community, ensuring everyone enjoys health and wellness.
  • Collaborate with primary care, behavioral health, and substance use treatment teams as part of the Community Health Team.
  • Foster relationships with individuals following psychosocial assessments, acting as the face of Springfield Medical Care Systems, Inc., and key point-of-contact to assist in achieving goals, identifying needs, and coordinating services.
  • Assist individuals who present at Neighborhood Connections following an assessment of their needs and goals.
  • Conduct follow-up assessments and engage individuals through motivational interviewing to identify priority needs and areas for intervention or follow up.
  • Identify next steps to address findings, including referral to an appropriate level of care either within Springfield Medical Care Systems, Inc. or an external service provider.
  • Provide brief interventions as necessary to address screening results, either directly or through referral.
  • Outreach to both organizations and individuals, including meeting people in their homes and visiting various service locations.
  • Assist in coordinating care for individuals, addressing barriers to and social influences on good health; assist with patient enrollment, schedule appointments, transportation, and follow up on care delivery.
  • Collaborate with local health and social service organizations to enhance services and address systemic barriers to health and social services.
  • Document all patient encounters; complete required reports; maintain electronic patient files, including referrals, releases, assessments, and other documents acquired on behalf of the patient.
Requirements:
  • You have experience with direct service provision, preferably in a healthcare or social service setting.
  • You possess a passion for working within your community, ideally having been a part of it in the past.
  • You are flexible, team-oriented, and willing to adapt to different situations.
  • You can effectively plan, organize, and coordinate access to health services independently.
  • You excel at communication, including writing, listening, and organizing information.
  • You demonstrate exceptional empathy and human interaction skills, aiming to improve individual and community health.
  • You are comfortable working with individuals experiencing challenges and difficulties in their lives and living circumstances.
  • You are an independent self-starter and strategic thinker eager to learn, improve, and grow.
  • You have experience using computer-based tools, including typing, web-based applications, and client-related databases.
  • You recognize technology's potential to support your work and drive ongoing evaluation toward better care.
  • You hold an unrestricted Driver's License.

As a Community Health Worker with Springfield Medical Care Systems, Inc., you will be part of a dynamic team dedicated to delivering high-quality care to our community. We offer a competitive salary range of $40,000 - $50,000 annually, depending on experience.
