  • Facilitates the assessment of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensures that patient needs are consistently evaluated and that care provided is tailored to individual requirements, appropriate, reasonable, meets home health eligibility standards, and aligns with physician directives.
  • Supervises and guarantees the formulation, execution, and revisions of the personalized patient care plan, as necessary.
  • Manages and records communications from physicians, clinicians, patients, and referral sources, relaying patient updates and new orders to clinicians. Utilizes coordination notes for documentation as needed.
  • Performs additional responsibilities as assigned.

Qualifications & Experience

  • Current RN licensure in the state of Texas
  • Current CPR certification is mandatory
  • Valid Driver's License, vehicle insurance, and access to a reliable vehicle
  • Previous experience in Home Health, particularly with OASIS, is highly preferred.
