Job Summary

SOME, Inc. is seeking a highly skilled Family Services Case Manager - Adult Services Coordinator to join our team. This role will provide case management and support services for adults and children living in Family Housing, developing and coordinating programming for adults on life skills and personal and professional development.

Key Responsibilities
  • Provide case management services utilizing a strength-based perspective and integrating motivational enhancement theory.
  • Collaborate with families and individuals to develop goals plans and monitor and report progress.
  • Identify, educate, advocate, and teach residents how to access and utilize community resources.
  • Implement clinical interventions and meet with families/individuals for case management at regular intervals.
  • Develop and coordinate activities for adults on life skills and personal and professional development.
  • Promote, market, and ensure attendance for all program activities and initiatives.
  • Identify resources and establish partnerships to assist in activities.
Requirements
  • High School Diploma/GED.
  • 5-8 years experience in social services, case management experience, and program development experience.
  • Prefer experience providing services to low-income families.
  • CPR/First-Aid certification and Graduate-level licensure is required for a Master's degree in Social Work or Counseling related program.
Working Conditions

This position is at Barnaby-Chesapeake House, which provides long-term housing for 32 families. The Family Services Case Manager - Adult Services Coordinator must be able to lift up to 20 pounds and requires looking at a computer screen for several hours a day.
