We are seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers at Pinnacle Home Care Inc.

About the Role

As a Physical Therapist with Pinnacle Home Care Inc., you will be responsible for providing high-quality physical therapy services to patients in their own homes. You will assess patients' conditions, develop personalized treatment plans, and deliver therapy to help improve mobility, reduce pain, and restore function.

Key Responsibilities
  • Assess patients' conditions and develop personalized treatment plans
  • Deliver physical therapy services to patients in their homes
  • Travel to patients' homes and adapt treatment plans to suit home environments
  • Ensure high-quality care and patient safety
  • Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
  • Teach, supervise, and counsel the family and client in the physical therapy program
  • Record evaluation data, treatments, and client's response to therapy intervention
  • Participate in case conferences, physical therapy staff meetings, and in-service programs
Requirements

To be successful in this role, you must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association. Additionally, the Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience.
