Responsibilities- Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to address these needs.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination of services.
- Supervise and provide clinical direction to home health aides and licensed practical nurses (LPNs) to ensure quality and continuity of services.
- Participate in on-call rotation and emergency call according to agency policy.
- Basic Cardiac Life Support (BCLS) certification within 30 days of employment start date.
- Registered Nurse (RN) licensure in the state of practice.
- Associate Degree or Registered Nurse Diploma.
