Responsibilities- Assessment: Conduct comprehensive assessments of patients' psychosocial situations, including medical, family, social, emotional, psychiatric, cultural, spiritual, financial, and environmental factors.
- Transition Planning: Collaborate with healthcare teams, patients, and families to develop and implement discharge plans, coordinating necessary services and resources.
- Resource Management: Utilize severity of illness-intensity of service criteria to determine appropriateness of admissions, transfers, continued stays, and discharges, and identify opportunities to reverse unnecessary admissions.
- Advocacy: Advocate for patients and families, communicating effectively with healthcare teams, payers, and community agencies to ensure timely and effective implementation of discharge plans.
- Experience: 3-5 years of relevant experience working with complex medical and psychosocial situations, with knowledge of community resources, insurance, and publicly funded programs.
- Education: Graduate of an accredited School of Nursing, with a BSN preferred.
- Licenses/Certifications: Current NYS RN license required, with Certification in PRI desired.
