Key Responsibilities: The Rehabilitation Therapist assesses and directly provides or oversees the delivery of therapy services to patients in their residences. When acting as the Home Care Case Manager (HCCM), the Rehabilitation Therapist is accountable for the provision and coordination of all elements of a patient's home care services. The therapist delivers care as prescribed by the physician, adheres to policies and procedures, and regularly communicates with their supervisor. The incumbent facilitates the development and execution of the care plan as directed by the physician. Additionally, the incumbent is responsible for all necessary documentation related to the care provided and communicates relevant information regarding the patient to all appropriate healthcare team members.

Minimum Level of Education Required: Bachelor's degreeAdditional requirements:- Type of degree: Bachelors - Graduate of an accredited Physical Therapy Program.- Area of study or major: Physical Therapy- Preferred educational qualifications: N/A- Position specific testing requirement: N/A
