About the Role

We are seeking a skilled and compassionate LPN/LVN to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Document patient progress and maintain accurate records, as required by the Company and/or facility.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
