Acute Care Admission Coordinator

We are seeking a skilled Clinical Professional to facilitate admissions, clinical intake assessments, and utilization review processes for Maple Heights Behavioral Health. This role plays a critical part in ensuring patients receive the most suitable level of care.
Responsibilities:- Medical Documentation Review
We require someone to meticulously review medical documentation for all admissions and obtain admission orders from physicians.
Emergency TransfersThis individual will be responsible for facilitating emergency transfers for patients presenting with emergency medical conditions beyond our facility's capabilities.
Referral ReviewsThe ideal candidate will conduct reviews of referrals from hospitals, facilities, and community agencies for medical and behavioral acuity, responding within our policy timeframes.
Care Level AssessmentsThey will conduct assessments to determine the appropriate level of care within our facility.
Collaboration and CommunicationThis role involves collaborating with other facility medical and psychiatric personnel to ensure accurate recommendations and admissions.
Admissions Process FacilitationOur team member will facilitate the admissions process for incoming patients, including assessment, consent, and inventory of patient belongings.
Insurance Benefits VerificationThey will perform insurance benefit verifications and secure initial pre-authorization for treatment and admission.
Patient Status MonitoringThroughout the admissions process, they will monitor patient status according to facility policy and physician orders.
Web-Based Programs UtilizationThis individual will utilize multiple web-based programs to track and review referrals.
Internal CommunicationThey will communicate projected admissions to designated internal representatives promptly.
Information Receipt and SchedulingThe ideal candidate will ensure that all clinical information from referral sources or patients is received prior to patient admission, when possible, and schedule and complete pre-admission assessments.
Medical Record DocumentationThey will possess skill in preparing and maintaining appropriate medical record documentation resulting in authorization at the requested level of care.
Pre-Authorization and AdmissionThis role requires completing initial pre-authorization for treatment and admission prior to admission, when possible, and within payor timeframe guidelines.
Patient Registration and ConsentOur team member will admit patients to the registration and accounting system, completing all admission and consent forms with the patient.
Professional Attitude and Prioritizing Patient SafetyThe ideal candidate demonstrates a positive, empathetic, and professional attitude toward customers and prioritizes patient safety.
