We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at SunCrest Home Health. As a Home Health Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities
- Provide technical skilled care to patients in their homes, in accordance with their individualized plans of care and state laws governing nursing practice.
- Instruct patients and their families on proper care and maintenance of a healthy environment, promoting independence and self-care.
- Observe, record, and report patient responses to treatment and changes in condition to the supervising nurse and/or physician on a daily basis.
Requirements
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
