About the Role

Virtua Health is seeking a skilled RN Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, diagnosing, planning, implementing, and evaluating patient care through outcome identification and evidence-based practice.

Key Responsibilities
  • Perform an assessment of the patient, leading to development, implementation, and evaluation of a plan of care.
  • Administer, monitor, and document therapeutic interventions and regimes.
  • Effectively manage rapidly changing situations.
  • Participate in clinical decision-making.
  • Update Surgeon Procedure Cards as needed.
  • Educate patients and families/responsible parties regarding disease processes, individual care needs, wellness, safety issues, etc.
  • Delegate and monitor care rendered by other members of the nursing department and healthcare team.
  • Demonstrate standard precautions and patient safety principles in practice.
  • Participate in orientation, education, and development of healthcare team.
  • Identify and participate in performance improvement activities.
  • Maintain professional and departmental level competencies.
Requirements
  • Must have a valid NJ RN License.
  • Approved licenses must be presented and maintained in accordance with the State of New Jersey licensing board.
  • Demonstrate knowledge of nursing skills, hospital practices, procedures, and standards.
  • Perfect skills and stay abreast of current nursing practice through ongoing education, journals, etc.
  • Must have strong communication skills.
  • Must be able to coordinate efforts of a team of care providers.
