We are seeking a skilled Case Management RN to join our team at Travel Nurses, Inc. in Sacramento, California. This is a 26-week assignment with 40 hours per week, 8-hour shifts, and a competitive salary.

Responsibilities
  • Provide case management services to patients with complex medical needs
  • Develop and implement individualized care plans to meet patient goals
  • Collaborate with healthcare teams to ensure seamless patient care
  • Communicate effectively with patients, families, and healthcare providers
  • Document patient progress and outcomes
Requirements
  • 1-2 years of recent experience in Case Management
  • Current RN licensure in California
  • Ability to work 40 hours per week, 8-hour shifts
  • Flexibility and adaptability in new environments
