Position Overview:
The Inpatient Care Coordinator, in partnership with the clinical team and healthcare providers, is responsible for overseeing discharge planning and coordination of care. This role is essential in promoting patient well-being, enhancing care outcomes, and ensuring efficient use of healthcare resources while minimizing payment denials for patients with intricate health challenges.
Key Responsibilities:
Assessment:
Conducts initial and ongoing assessments to evaluate the patient's condition and determine appropriate care coordination and discharge planning needs within the inpatient environment.
Planning:
Develops a targeted discharge plan for high-risk patients based on identified needs.
Intervention/Evaluation:
Works collaboratively with the clinical team and healthcare providers to implement the planned discharge strategy for patients.
Performance Improvement:
Remains informed about evolving organizational policies, emergency department practices, care management trends, regulatory requirements, and third-party payer guidelines related to clinical care and discharge planning.
Qualifications:
- Bachelor's degree in a relevant field; advanced knowledge in a specialized area preferred.
- BSN is preferred; ADN is required.
- 3-5 years of relevant experience; prior experience in inpatient case management or discharge planning is advantageous.
- Must possess a Maryland Registered Nurse License or have intent to obtain licensure if licensed in another state.
- Case Management certification is preferred within 3-5 years of employment.
