Weekend Registered Nurse Opportunity

JourneyCare Home Health is seeking a dedicated and compassionate Registered Nurse (RN) to join our team in Evanston, IL. As a Weekend RN, you will provide high-quality patient-centered care to individuals in their homes over the weekends. Your focus will be on delivering exceptional nursing services, coordinating with healthcare professionals, and supporting patients and their families through compassionate care.

Key Responsibilities:
  • Cover a designated Evanston, IL territory, including zip code 60201 and surrounding areas
  • Provide visits as assigned by the Clinical Services Manager, including assessment, determination of eligibility, admissions processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact patient and family needs and coordinate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
  • Provide visits to assess patient needs and update the plan of care
  • Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician
  • Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely
Requirements:
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience, preferably in intensive care, rehabilitation, or medical surgical nursing
  • Home health experience preferred
  • Possess and maintain valid CPR certification
  • Must have reliable transportation, current driver's license, and appropriate automobile insurance
