As a Critical Care Transition Specialist, you will provide specialized care for patients transitioning from critical care to general medical-surgical units. Your responsibilities will include assessing, monitoring, and treating patients, collaborating with the interdisciplinary team, and ensuring optimal outcomes.

Key Responsibilities:
  • Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status
  • Administer medications, perform procedures, and collaborate with interdisciplinary teams
  • Educate patients and families on post-intensive care recovery, medication management, and self-care techniques
  • Communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes
Requirements

We are seeking a highly motivated and experienced Registered Nurse with a strong background in critical care. The ideal candidate will have:

  • Current RN license
  • BLS (Other Certifications as required by facility)
  • Two years of recent Step Down experience
  • Strong communication and adaptability skills
