About the Role

We are seeking a highly skilled Physical Therapist to join our team at Knute Nelson. As a Physical Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services in a home health setting.

Key Responsibilities
  • Evaluate and Assess Patients: Conduct thorough evaluations and assessments of patients to identify their physical therapy needs and develop personalized treatment plans.
  • Develop and Implement Treatment Plans: Create and implement effective treatment plans that meet the unique needs of each patient, with the goal of improving their functional abilities and quality of life.
  • Provide Quality Care: Deliver high-quality physical therapy services to patients in their homes, with a focus on promoting independence, mobility, and overall well-being.
  • Communicate Effectively: Communicate effectively with patients, families, healthcare providers, and other members of the interdisciplinary team to ensure seamless care coordination and optimal patient outcomes.
  • Document and Record Patient Information: Accurately and thoroughly document patient information, including treatment plans, progress notes, and outcomes, in accordance with agency policies and procedures.
Requirements
  • Licensure and Certification: Must be licensed as a Physical Therapist in the state of Minnesota and obtain CPR certification within 6 months of hire.
  • Education and Experience: Must have a valid driver's license and use of an insured personal vehicle, as well as the ability to read, write, and speak English well enough to communicate with staff, clients, and visitors.
