Discharge Planning Specialist

As an RN Discharge Planner-Weekends, your key responsibilities will include:
- Carefully assessing patients' biophysical, psychosocial, environmental, discharge planning needs, and financial status.
- Working closely with patients, families, and healthcare teams to coordinate care and services.
- Ensuring that patients receive the necessary support and resources to achieve their goals.
- Collaborating with third-party payers to resolve problems and issues affecting the continuity of patient care.
Requirements:
To be successful in this role, you will need to have a strong understanding of nursing practices and procedures. You must also be knowledgeable about resources available in the community for use by patients, families, and the healthcare team. A current RN License issued by the Oklahoma State Board of Nursing or a current multistate Compact RN License (eNLC) is required. Additionally, a Basic Life Support issued by the American Heart Association is required upon hire.
Estimated Salary: The estimated salary for this position is $92,000 per year, depending on experience. Ou Health offers a comprehensive benefits package, including PTO, 401(k), medical, and dental plans.
