Home Health Nurse Case Manager

Become a part of our caring community and help us put health first as a skilled nurse in Home Health. Our team is dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
Responsibilities:- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Confer with physicians to develop treatment plans based on patient assessment.
- Coordinate care with various healthcare personnel and oversee the work of Home Health Aides and Personal Care Workers.
- Report patient care/condition/progress to physicians and Clinical Manager.
- Prepare appropriate medical documentation on all patients.
- Diploma, Associate, or Bachelor Degree in Nursing
- A minimum of one year of nursing experience preferred
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
