About the Role
We are seeking experienced Nurse Practitioners and Physician Assistants to join our team as In-Home Healthcare Professionals. This role involves providing acute care in the home setting, conducting comprehensive health assessments, diagnosing complex illnesses, and delivering therapeutic interventions while operating in a mobile unit. Our professionals prioritize compassionate, patient-centered care and support professional development within a rapidly growing healthcare organization.
Key Responsibilities:
• Conduct comprehensive health assessments and diagnose complex illnesses
• Deliver therapeutic interventions, such as splinting, suturing, wound care, and minor procedures
• Accurately document patient encounters and ensure accuracy
• Analyze test data to diagnose, treat, and arrange follow-up care for continuity
• Educate patients and families on treating acute illnesses and managing their health and well-being
• Communicate effectively with patients, family, medical power of attorney, primary care provider, and individuals involved in patient care
• Adhere to clinical and safety standards, protocols, and performance metrics
• Provide care with compassion, empathy, and cultural competency
• Maintain positive relationships with partners and remote teams
Requirements:
• Minimum of two years of experience as an APP in Emergency Department, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary, or relevant areas
• Advanced degree from an accredited institution
• Complete required drivers training upon hire
• Ability to lift and carry equipment up to 50 pounds
• Flexible to work evenings, weekends, and holidays as needed
• Must be licensed as an APP in VA
