Job Description:
At Sharecare, we are committed to making high-quality care more accessible and affordable for everyone. As a Hospital Diabetes Clinician, you will play a critical role in educating patients with diabetes in both inpatient and outpatient settings.
You will work closely with physicians and a network of hospital clinicians to identify complex inpatients who require diabetes education and support. Your responsibilities will include providing education personally and/or guiding the staff nurse on required learning needs prior to discharge, including changes to medications, dietary orders, or other areas of expertise.
Schedule:
Monday - Friday, 8am - 4:30pm ET. On-call one weekend a month.
Location:
Ballad Health Hospital in Bristol, TN. Must be comfortable traveling to various other hospitals in the Tri-Cities area.
Essential Job Functions:
Partner directly with the Program Director, physicians, Resource/Utilization/Case Management, diabetes staff, and other hospital staff to achieve program objectives, quality measures, enhance clinical outcomes, and customer satisfaction for both the outpatient service line.
May also assist in other responsibilities such as marketing and community services.
Assess patient's understanding of disease process; chronic complications; standards of care and current areas requiring clinical intervention via interview and self-assessment and/or medical record and assists patient to set self-care goals to improve disease management.
Provide group and individual education; implements interventions within their scope of practice, such as medical nutrition therapy, self-blood glucose monitoring, medication/injectable, insulin pump therapy (if certified) to improve disease management and meet the hospital, Sharecare, and any regulatory agency requirements.
Works with referring providers to identify the diabetes management needs of patients/families.
Works with interdisciplinary team to facilitate case management of complex diabetes patients.
Specific Skills/ Attributes:
Flexibility to work days, evenings, and some weekends.
Ability to learn new systems and work processes.
Qualifications:
Licensed Registered Nurse or Registered Dietitian required.
Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) preferred.
Minimum 2+ years of related experience in a hospital or physician office setting preferred.
Appropriate professional licensure/registration required.
