Job Summary:

We are looking for a dedicated Speech Language Pathologist to provide high-quality care to our clients in their homes. As a member of our multidisciplinary team, you will work closely with other healthcare professionals to develop and implement individualized care plans that promote the health, safety, and independence of our clients.

Responsibilities:
  • Conduct thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
  • Develop and implement treatment plans based on client needs and physician orders.
  • Provide education and instruction to clients, family members, or caregivers on rehabilitative care and activities necessary to promote client health and safety.
  • Accurately document client care management and services provided.
Requirements:
  • Current license as a Speech Language Pathologist in Pennsylvania.
  • Certificate of clinical competence in speech from the American Speech and Hearing Association or equivalent education and work experience.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
