Hospital Medicine
Perioperative and Recovery Excellence Team
Our Hospital Medicine division is seeking a dedicated and passionate physician to oversee patient care within our innovative hospital-at-home initiative and perioperative surgical home clinic.
As a member of this dynamic team, you will gain expertise in perioperative medical management and telehealth practices, focusing on delivering acute care services in the comfort of patients' homes, thereby reducing the need for hospital admissions.
You will receive support from a committed medical director and clinical staff dedicated to the program's success. Additionally, collaboration with fellow hospitalists will enhance your professional experience.
We are looking for individuals who prioritize patient-centered care, maintain a safe care environment, and demonstrate adaptability and flexibility in their practice.
Compensation Range:
$306,000 to $340,000 annually based on experience and performance with quality incentives.
_Perioperative Surgical Home Program Overview_
Program Summary:
The ideal candidate will conduct preoperative risk assessments and optimize patients' health prior to surgical procedures, ensuring they are in optimal condition for surgery.
This role requires collaboration with a multidisciplinary team to provide patient-focused care, improve surgical outcomes, and enhance the overall patient experience.
Key Responsibilities:

Preoperative Risk Assessment and Optimization:
- Perform thorough preoperative evaluations, including comprehensive medical histories and physical assessments.
- Order and interpret necessary diagnostic tests to ensure patients are in peak health before surgery.
- Develop tailored preoperative care plans in partnership with surgeons, anesthesiologists, and other healthcare professionals.
Risk Management:
- Identify and manage medical conditions or risk factors that may impact surgical outcomes.
- Implement strategies to mitigate surgical risks and complications, focusing on optimizing chronic conditions and addressing acute issues.
- Provide recommendations for preoperative enhancements, including nutritional support, smoking cessation, and exercise plans.
Care Coordination:
- Collaborate with surgeons, anesthesiologists, nurses, and other healthcare professionals to ensure smooth preoperative processes.
- Facilitate communication among patients, families, and the healthcare team to ensure clarity and understanding.
- Ensure all preoperative preparations, including medications and lifestyle modifications, are completed and comprehended by patients.
Quality Improvement:
- Engage in quality improvement initiatives aimed at enhancing patient outcomes and program efficiency.
- Collect and analyze data related to preoperative care processes and patient outcomes.

_Home Recovery Care Program Overview_
Program Summary:
The ideal candidate will deliver acute medical care to patients in their homes, ensuring they receive high-quality, hospital-level care in a familiar setting.
This role necessitates close collaboration with a multidisciplinary team to manage various acute and chronic conditions, improve patient outcomes, and enhance satisfaction.
Key Responsibilities:

Patient Assessment and Care:
- Conduct comprehensive initial assessments of patients referred to the Hospital at Home Care Program.
- Develop and implement individualized care plans based on patient needs and medical history.
- Provide acute care management for conditions typically requiring hospitalization.
- Monitor patient progress through daily telehealth consultations and follow-up visits.
- Adjust care plans as necessary based on patient responses and changes in condition.
Care Coordination:
- Collaborate with a multidisciplinary team, including nurses, physical therapists, and social workers, to deliver coordinated care.
- Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.
- Coordinate care transitions, including hospital admissions and discharges, to ensure continuity of care.
Patient Education and Support:
- Educate patients and families about their conditions, treatment plans, and self-care strategies.
- Provide support and resources to help patients manage their health and prevent complications.
- Address any concerns or questions from patients and families promptly and compassionately.
Professional Development:
- Stay updated with the latest advancements in home-based healthcare and hospital-at-home models.

Qualifications
Requirements include:
- Medical Degree
- Completed ACGME Residency Training
- Board Certification - American Board of Internal Medicine
- ACLS, Washington State Medical License, and DEA in good standing
Pay Range
$161.83 /hour
