Job Title: Registered Nurse

Location: Osage Rehabilitation and Health Care Center, Osage, IA

Shifts: Full-Time and Part-Time, 6 AM - 2 PM, with every other weekend required

Job Summary:

As a Registered Nurse at Osage Rehabilitation and Health Care Center, you will be responsible for providing high-quality nursing care to our residents. This role involves assessing, planning, implementing, and evaluating patient care in a long-term care setting. You will work closely with other healthcare professionals to ensure the best possible outcomes for our residents.

Responsibilities:
  • Patient Care:
    • Perform comprehensive assessments of patients, including physical, emotional, and social needs.
    • Develop and implement individualized care plans based on patient assessments.
    • Administer medications and treatments as prescribed by physicians.
    • Monitor and record patient progress, vital signs, and symptoms.
    • Provide wound care, catheter care, and other necessary medical procedures.
  • Coordination and Communication:
    • Collaborate with physicians, therapists, and other healthcare team members to coordinate care.
    • Communicate effectively with patients and their families regarding care plans and progress.
    • Document patient interactions and care in accordance with facility policies and state regulations.
  • Patient Advocacy:
    • Advocate for patient rights and ensure their needs are met in a compassionate and respectful manner.
    • Address and resolve patient concerns or complaints promptly.
  • Compliance and Safety:
    • Adhere to all regulatory requirements and facility policies.
    • Ensure the safety and cleanliness of the patient environment.
    • Participate in infection control practices and emergency procedures.
  • Education and Training:
    • Educate patients and families about health conditions and care plans.
    • Stay updated with current nursing practices and regulations through ongoing education and training.
  • Administrative Duties:
    • Assist with admissions, discharges, and transfers of patients.
    • Participate in facility meetings and contribute to policy development and review.
Qualifications:
  • Education and Licensure:
    • Current and valid Registered Nurse (RN) license in the state of Iowa.
    • Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Experience:
    • Prior experience in a long-term care or rehabilitation setting is preferred but not required.
  • Skills and Competencies:
    • Strong clinical assessment and decision-making skills.
    • Excellent communication and interpersonal skills.
    • Ability to work collaboratively as part of a healthcare team.
    • Proficient in electronic medical record (EMR) systems and other healthcare technologies.
    • Compassionate and patient-centered approach to care.
Additional Requirements:
  • Ability to work weekends and holidays as required.
  • Must pass a background check and drug screening.
  • CPR and First Aid certification preferred.
Working Conditions:
  • Full-Time: 40 hours per week, 6 AM - 2 PM, including every other weekend.
  • Part-Time: Variable hours, 6 AM - 2 PM, including every other weekend. Physical requirements include the ability to lift, transfer, and assist patients as needed, and to stand or walk for extended periods.
