Job Summary:

The Healthcare Navigator will play a crucial role in coordinating care for vulnerable individuals, ensuring they receive whole-person wellness and recovery services.

This position requires strong communication and interpersonal skills, as well as the ability to work effectively with healthcare professionals, community organizations, and government agencies.

Responsibilities:

  • Conduct outreach and education to identify individuals in need of care coordination
  • Collaborate with healthcare professionals to develop and implement individualized care plans
  • Engage eligible members in care coordination services
  • Provide advocacy and support to ensure seamless transitions between healthcare settings
  • Monitor treatment adherence and provide health promotion training

Requirements:

  • Bachelor's degree in a related field (e.g., social work, public health)
  • 1 year of experience working with vulnerable populations (e.g., homeless, severely mentally ill, substance use disorder diagnosed)
  • Class C driving license required
  • Knowledge of behavioral health, housing/homelessness, and social determinants of health
  • Ability to communicate effectively with diverse stakeholders

Working Conditions:

This position may require occasional evenings and weekends, as well as work in inclement weather or unsanitary conditions.

Salary: $60,000 - $70,000 per annum
