Responsibilities

As a Speech Language Pathologist at HarmonyCares, your key responsibilities will include:

  • Assessing and evaluating patient needs and developing personalized care plans.
  • Providing speech therapy services, including assessment, evaluation, procedures, teaching, and training activities.
  • Reporting patient status to clinical supervisors and referring physicians.
  • Documenting daily changes and updates to patient care plans, including discharge planning.
  • Attending staff meetings, team conferences, and educational in-services.
  • Performing OASIS time point assessments and submitting recertification paperwork.
Qualifications

To be successful in this role, you will need:

  • A current, unencumbered state professional license.
  • A current driver's license and proof of insurance.
  • At least one year of experience as a home care Speech Language Pathologist, with competence in performing comprehensive assessments and OASIS-C.
  • The ability to make sound professional clinical judgment.
  • The ability to assess and document patient needs and formulate individualized patient care plans.
  • Proficiency in clinical skills and Microsoft Office.
