Job SummaryThe Physical Therapist Assistant (PTA) will provide physical therapy services to patients in their homes, communicating with other disciplines and directing non-professional home health aide staff in the provision of personal care. The PTA will submit completed physical therapy visit notes and participate in discharge planning, as well as attend staff meetings and educational in-services.
Responsibilities- Assess and document patient needs and formulate individualized patient care plans
