Job Title: Care Transitions Coordinator Home Health

About the Role:

We are seeking a highly motivated and experienced Care Transitions Coordinator to join our team at Enhabit Home Health & Hospice. As a Care Transitions Coordinator, you will play a critical role in ensuring seamless transitions for our patients from acute care settings to home-based care.

Key Responsibilities:

  • Assist patients in navigating post-acute care, promoting positive outcomes and referral source satisfaction.
  • Develop patient-centered transition plans, integrating evidence-based clinical guidelines and protocols.
  • Represent Enhabit Home Health & Hospice in strategic relationships with health systems, hospitals, and physician groups.

Requirements:

  • Registered Nurse (RN) or Physical Therapist (PT) with a graduate degree from an approved school.
  • Two years of demonstrated field experience, with a strong understanding of federal, state, and local laws governing home health and hospice operations.
  • Basic technology skills, including operation of a mobile device.

Preferred Qualifications:

  • Three years of field experience.
  • Previous experience in home health or healthcare sales.
