Responsibilities- Provide high-quality clinical care to medically fragile children in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and families on disease management and self-care
- Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing
