Join a dynamic team in a diverse environment dedicated to supporting individuals in their journey towards mental health and addiction recovery. As a key member of a multidisciplinary team, you will engage in comprehensive assessments, deliver psychoeducational sessions, and provide individual, family, and group therapy to patients requiring substance abuse and dual diagnosis treatment at Carle BroMenn Addiction Recovery Center. Collaborate closely with hospital personnel to ensure treatment options align with patient needs and assist in coordinating transfers to external facilities when necessary. Provide short-term therapeutic interventions for patients facing addiction and/or mental health challenges.

Qualifications:

EDUCATIONAL REQUIREMENTS
Master's Degree in Social Work, Counseling, Psychology, or a related field.

CERTIFICATION & LICENSURE REQUIREMENTS
Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) in Illinois upon hire.

EXPERIENCE REQUIREMENTS
Minimum of 2 years of experience in treating individuals with a history of substance use.

SKILLS AND KNOWLEDGE
Expertise in clinical assessment related to substance use and recovery support.

Key Responsibilities:

  • Conduct psychosocial assessments and provide individual, family, and group therapy for inpatients, PHP, and IOP as per schedule.
  • Facilitate psychoeducational groups for inpatient, PHP, and IOP as required.
  • Manage clinical case assignments, including patient transfers, referrals, and discharges.
  • Complete assessments, treatment plans, and progress notes for all assigned patients.
  • Participate in treatment team meetings, providing clinical insights regarding patient care.
  • Deliver ongoing therapy services tailored to the specific needs of patients.
  • Collaborate with patients to develop personalized treatment plans with clear goals and objectives.
  • Ensure timely documentation in medical records as per departmental policies.
  • Communicate with referring providers about patient diagnosis, treatment plans, and progress, with patient consent.
  • Engage with the Treatment Team weekly to review the clinical care of patients.
  • Assess the quality of patient and family support networks within the community.
  • Provide case management and referrals to appropriate treatment or community agencies.
  • Maintain up-to-date information on community resources and agencies for patients and families.
  • Educate patients, interns, medical students, Nurse Practitioners, Physician Assistants, and Social Workers.
  • Conduct psychosocial assessments and treatment related to substance use disorders and discharge planning.
  • Consult with multidisciplinary team members regarding assessment, treatment, and discharge planning for patients with substance use disorders.
  • Inform RNs, leadership, and physicians of relevant clinical information obtained through therapies or assessments.
