We are seeking a skilled Per Diem Nurse to join our team at Good Shepherd Community Care. As a Per Diem Nurse, you will provide high-quality patient care in the comfort of their own homes and residential facilities throughout Greater Boston and parts of Metrowest.
Job Responsibilities:- Participate in the interdisciplinary team to provide clinical assessments of patients' ADLs, functional, psychological, and environmental needs.
- Implement and monitor patient care, including managing discomfort and symptom relief.
- Utilize specialized nursing skills and agency resources related to end-of-life care in accordance with the Nursing Care Coordinator's plan of care and Good Shepherd's policies and quality assurance guidelines.
- Consult with and educate patients and families about disease process, self-care techniques, end-of-life, and ethical concerns.
- Collaborate and coordinate patient care with various healthcare facilities and professional healthcare providers.
- Licensed Registered Nurse in the State of Massachusetts.
- Sensitivity and understanding of end-of-life issues.
- Experience in community health, hospice, oncology, or related field preferred.
- Demonstrated skills in assessment, pain management, and symptom control.
- Excellent organizational and time management skills.
- Exceptional communication skills.
