Family Nurse Practitioner

We are seeking a highly skilled and motivated Family Nurse Practitioner to join our team at ALLMED Staffing. As a Family Nurse Practitioner, you will be responsible for providing high-quality patient care in a home health setting.
Key Responsibilities- Conduct comprehensive health risk assessments, including history and physicals, in patients' homes or other health facilities.
- Document patient encounters accurately, efficiently, and in a timely manner using electronic medical records.
- Ensure all necessary consent forms are signed and dated.
- Interact frequently with patients, coders, schedulers, and supervising physicians.
- Provide education to patients and their families on chronic medical conditions, preventative care, and medication adherence.
- Complete referrals to case management, nursing, pharmacy, and other services as needed.
- Participate in clinical meetings and compliance trainings, primarily web-based, with occasional on-site training.
- Obtain samples for diagnostic screening tests, such as finger sticks for HgbA1C, LDL, and Lead levels, and urine analysis as required.
- Review patient records for past and present health issues.
- Collaborate with supervising physicians and other healthcare team members.
- Interpret laboratory data and diagnostic testing to assist in medical condition diagnosis.
- Perform ADL and Fall Risk evaluations, Depression and Nutritional Health Screening, BMI evaluations, and medication reconciliation as part of assessments.
- Perform other duties assigned by supervising physicians, such as diabetic retinal exams, bone density testing, and examinations using specialized equipment.
- Training as a Family NP is required; adult NP considered if no pediatric patients will be seen.
- Board certification by the ANCC or AANP is required.
- Active and unrestricted license in the state of position opening is required.
- New graduates will be considered.
- Candidates must be able to be credentialed by Medicare, Medicaid, and other private insurance companies.
- Ability to work independently and as part of a team.
- Excellent clinical assessment and analytical skills.
- Effective leadership and interpersonal skills.
- Excellent verbal and written communication skills.
- Proven ability to perform comprehensive physical exams and diagnose chronic diseases in adults.
- Ability to recognize conditions, characteristics, and behaviors that place seniors and children at risk.
- Proven ability to pursue feedback, continuing education, and professional development.
- Ability to travel up to 100 miles from home zip code.
