Position Overview
The Healthcare Case Navigator plays a vital role in assisting families to access essential community resources and services. This position is integral to maintaining population health initiatives and facilitating care coordination efforts.
Work Schedule
This is a full-time position, requiring 40 hours per week, Monday through Friday, from 8 AM to 5 PM.
Role Responsibilities
- Actively engages with families to encourage participation in various case management programs by clearly communicating goals, benefits, and interventions.
- Gathers information for personalized assessments to determine eligibility for case management services and refers individuals when necessary for licensed clinical intervention.
- Provides tailored navigation support to assist families in maneuvering through the healthcare system, addressing social determinants of health, and accommodating cultural and linguistic needs.
- Coordinates outreach to primary care providers, specialists, and home care services to facilitate referrals and enhance patient self-management through education and resource access.
- Manages incoming calls, schedules appointments, and assists families in resolving immediate needs efficiently.
- Maintains accurate records and case files, documenting personal information, services rendered, progress towards goals, and significant changes.
- Participates in the orientation and ongoing education of staff and students as appropriate.
Qualifications
Education: A Bachelor's degree in healthcare, public health, or a related clinical field is preferred.
Licensure: A valid driver's license and proof of auto insurance are required.
Certifications: Active Basic Life Support (BLS) certification is required (may be obtained upon hire).
Skills: A solid understanding of Medicaid and other regulatory standards is essential.
Experience: A minimum of 2 years of experience in a patient-facing role within the healthcare sector is required.
