About the Role:

As a Community Health Liaison Specialist, you will play a critical role in connecting patients and families with essential health and social services. You will work collaboratively with medical providers, primary care teams, and community organizations to provide personalized support and guidance.

As a Community Health Liaison Specialist, you will have the opportunity to make a meaningful impact on the lives of individuals and families. You will be responsible for conducting home visits, providing health education and counseling, and advocating for patients' needs. You will also participate in ongoing education and training to enhance your knowledge and skills.

We are committed to providing a comprehensive compensation package that includes a salary of $22.14 per hour, which translates to a total stipend of $7,500 over the 550-hour training period. This stipend will be paid out on a semi-monthly basis.

Requirements:
  • High school diploma or equivalent required.
  • Valid driver's license and reliable transportation.
  • Ability to pass a background check.
