Advanced Practice Registered Nurse

Theoria Medical is seeking a highly skilled and dedicated Advanced Practice Registered Nurse (APRN) to join our team as a Primary Care Provider. In this role, you will be responsible for providing high-quality, patient-centered care to residents in skilled nursing facilities (SNFs) across the country.
Key Responsibilities- Provide comprehensive primary care services to patients residing in SNFs, including rounding, prescribing medications, and treatments.
- Document patient care and encounters in a timely and accurate manner using Theoria's proprietary electronic health record (EHR) system, ChartEasyTM.
- Collaborate with facility nursing staff to ensure seamless care coordination and address acute/emergent issues that arise.
- Participate in ongoing education and training to stay up-to-date on the latest medical information and best practices.
- Current licensure as an APRN in the United States.
- Experience working in a SNF or post-acute care setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Reliable transportation and a valid driver's license.
