Job Title: Registered Nurse (Home Health)

LHC Group is seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Cedartown, Georgia.

About the Role:

As a Registered Nurse in Home Health, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort. Your expertise will be instrumental in developing and implementing personalized care plans, collaborating with healthcare professionals, and ensuring seamless transitions between care settings.

Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans in collaboration with the patient, family, and healthcare team.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress, outcomes, and any changes in condition, ensuring accurate and timely reporting.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Current RN licensure in the state of Georgia.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
