Job Summary

As a Nurse Care Coordinator at Prisma Health, you will play a vital role in delivering exceptional patient care and promoting a safe, timely, and effective healthcare environment. This position requires a compassionate and skilled Registered Nurse to provide clinical direction for all aspects of patient care in an acute inpatient setting.

About the Job:

  • Provides clinical direction for all aspects of patient care, fostering patient/family centered care.
  • Demonstrates clinical competency utilizing evidence-based practices/research in all areas of the nursing process: assessment, planning, implementation, evaluation, and outcomes.
  • Consults with clinical advisors and peers to ensure compliance with organizational policies and procedures, regulatory agency requirements (JCAHO, DHEC, CMS), and the South Carolina Nurse Practice Act.

Key Responsibilities:

Assessment
  • Collects data and completes nursing assessments using appropriate area and/or age-specific standards/policies and tools.
  • Recognizes subtle or actual changes in patients' conditions and patient responses to interventions.
  • Validates priorities with healthcare team members and documents appropriately.
Planning & Outcomes
  • Develops an age-appropriate, culturally sensitive, individualized plan of care that reflects current evidence-based practice and addresses continuity of care.
  • Makes referrals based on assessment (e.g., skin, nutrition, social services).
  • Updates plans based on changes in patients' conditions; communicates changes to other team members.
Implementation
  • Provides direct care to assigned patients in a safe, timely, and cost-effective manner, consistent with established plans of care and identified patient priorities.
  • Delegates nursing tasks as appropriate to members of the healthcare team.
  • Ensures smooth, safe hand-offs of assigned patients to other departments and/or caregivers.
Evaluation
  • Evaluates plans of care for effectiveness and revises as needed.
  • Documents progress toward expected outcomes and refers unresolved problems to appropriate healthcare team members.
Patient/Family Education/Teaching
  • Identifies and assesses patient/family learning needs and teaches accordingly.
  • Communicates patient/family education/teaching needs to others in the healthcare continuum.
Professional Accountability
  • Evaluates own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations, and current specific knowledge.
  • Maintains professional licensure and completes required mandatory education in a timely manner.

Required Skills and Qualifications:

  • Associate's Degree - Nursing
  • One year of experience in Nursing as a Registered Nurse
  • SC Licensed RN - Registered Nurse

Estimated Salary Range: $65,000 - $80,000 per annum, depending on experience. Located in Greenville Memorial Med Campus, 1008 Greenville Memorial Hospital, Department ICU Medical.
