Qualifications Required

• Active Pharmacy License in the relevant state.
• Not listed on the DEA Excluded Parties List.
• Certification in Immunization from an accredited organization.
• Designated as the pharmacist in charge on the state pharmacy license.
• Completion of necessary documentation for state PMP access where applicable.
• Bachelor of Science in Pharmacy or Pharm. D. degree.
