Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Skylake Post Acute in Thornton, Colorado. As a rehabilitation specialist, you will provide direct nursing care to our residents and supervise the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs) to ensure the highest quality of care is maintained.

Responsibilities
  • Provide direct nursing care to residents, including administering medications, wound care, and vital sign monitoring.
  • Supervise CNAs to ensure they are providing high-quality care and adhering to our facility's policies and procedures.
  • Conduct regular assessments of residents' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians and therapists, to ensure comprehensive care for our residents.
  • Participate in quality improvement initiatives to enhance the overall quality of care at Skylake Post Acute.
Requirements
  • Nursing degree from an accredited college or university or an approved LPN/LVN program.
  • Current, unencumbered license to practice as an LPN in Colorado.
  • CPR certification.
  • Ability to pass a criminal background check and Colorado CAPS background check.
Work Environment

This is a full-time, part-time, or PRN position with a competitive salary range of $30-$38 per hour. As a member of our team, you will have the opportunity to work in a dynamic and supportive environment that values the well-being of our residents and staff.
