About the Role

RiverSpring Health Plans is seeking a skilled Nurse Care Manager to join our team. As a Nurse Care Manager, you will play a critical role in collaborating with members and their families to maintain independent living situations and ensure consistent care along the healthcare continuum.

Key Responsibilities

  • Assess and closely monitor members' needs and status
  • Authorize covered services and coordinate care regardless of payer
  • Collaborate and communicate with members/families/caregivers, primary care practitioners, and the interdisciplinary team

Requirements

  • Current NYS RN License and registration
  • Bachelor of Science degree in Nursing required with a minimum three (3) years of RN experience
  • Clinical expertise in geriatrics preferred; managed long-term care experience preferred
  • Bilingual skills in Russian required

Benefits

  • $95,000-$110,000 Annually
