**Job Summary**
We are seeking a compassionate and dedicated professional to provide psychosocial assessments and medical social services for patients and their families. As a Palliative Care Social Worker, you will play a vital role in facilitating patient-family meetings, providing emotional support, coordinating educational services, and acting as a liaison between different care teams and community services.
**Responsibilities**
* Conduct thorough psychosocial assessments to identify patient and family needs
* Develop and implement individualized care plans to address patient and family concerns
* Provide emotional support and counseling to patients and their families
* Coordinate educational services to promote patient understanding and self-advocacy
* Serve as a liaison between healthcare teams and community services to ensure seamless care coordination
**Requirements**
* Master's degree in Social Work
* LCSW licensure
* CPR certification
* Experience in palliative care
