Case Manager for Traumatic Brain Injury and Aging Services

We are seeking an experienced Case Manager to join our team at TLC Support Services LLC. In this role, you will be responsible for coordinating care for individuals with traumatic brain injury and aging population, developing and implementing individualized service plans, and working closely with community-based service providers to ensure seamless care delivery.
As a Case Manager, you will have the opportunity to make a meaningful difference in the lives of our clients, supporting them to maintain their independence and achieve their goals. You will work closely with a multidisciplinary team to ensure that our clients receive comprehensive care and support.
The ideal candidate will possess a Bachelor's degree in Social Work or a related field, with a minimum of three years of experience in case management and service coordination. Strong communication and interpersonal skills, as well as the ability to work independently, are essential requirements for this position. Salary Range: $20-$23 per hour
