Job Qualifications- Must possess a current LPN license or be eligible for licensure in the State of Florida.
- Completion of an accredited Nursing program focused on Licensed Practical Nursing.
- A minimum of one year’s experience in Hospice, Medical-Surgical, or other acute care settings is required.
- A valid driver's license and a clean driving record are essential for this role.
- Exemplary professionalism in communication with patients, their families, colleagues, physicians, and other healthcare personnel.
- On-Call duties are mandatory.
- Preferred: Certification from a 30-hour IV training course.
- Additional IV certification will be provided by the facility post initial certification.
- Deliver compassionate care to patients facing terminal illnesses and their families.
- Conduct and document thorough assessments.
- Communicate any changes to the supervising RN promptly.
- Provide education to patients and their families.
- Clarify the implications of diagnoses/prognoses and treatment options based on information from the primary nurse and/or physician.
- Collaborate with an Interdisciplinary Team to create a Plan of Care that reflects patient and family-centered goals.
- Utilize critical thinking skills effectively.
- Act as a representative of Unavailable, employing effective communication methods and skills.
- Engage in quality improvement initiatives.
- Maintain professional and positive working relationships.
- Advocate for a holistic approach to the care of terminally ill patients.
