Per Diem Case Manager

Mass General Brigham is seeking a skilled RN Case Manager to join our team on a per diem basis. As a key member of our Case Management Department, you will play a vital role in ensuring the highest quality care for our patients.
Job Summary:The RN Case Manager will be responsible for planning, developing, and implementing the operations of the Case Management Department. This includes coordinating care activities, quality improvement, and case management to meet the requirements of The Joint Commission, state, and local regulatory agencies.
Key Responsibilities:- Assist in planning, developing, and organizing the operations of the Case Management Department
- Coordinate care activities, quality improvement, and case management to meet regulatory requirements
- Obtain pre-certification from third-party payors as required
- Perform daily utilization review activities to assess appropriateness of admission and continued stays
- Communicate with the interdisciplinary team and community resource representatives to plan for patient transitions to post-hospital care
- Establish and implement safe, comprehensive discharge plans to meet individual patient needs
- Coordinate appeal process for reimbursement denials with Medical Staff, Billing, and Medical Records
- Registered Nurse with previous case management experience required
- Successful completion of Case Management Certification Program preferred
- Prior work in a community health, home health, or acute care or critical access hospital setting
- Current, unencumbered license to practice in this state
